    In healthcare management, effective communication and streamlined coordination among team members are paramount for the success of any project. A team roster and communication plan serve as fundamental tools for a project manager, providing a centralized and organized platform to document essential information about team members. This includes their roles, responsibilities and contact details, ensuring that everyone involved is on the same page regarding their contributions to the project.  

    This downloadable team roster and communication plan template will help you define team member roles (e.g., executive sponsor, project owner, project manager, contributing team members) and receive sign-off before a project begins. Once completed, share it with all points of contact (POC), including project leaders. If resources are stretched thin, share with project team members’ immediate management, as well, to determine next steps.
